Mail (provide name and address) - Please note, when requesting a Baptism or Marriage Certificate, proper identification of the person whose name is on the Certificate must be emailed to [email protected] before certificate can be mailed.
Mail to a Church or a School (provide name and address) - If you are requesting a Baptism Certificate because you are getting married in a Catholic Church or enrolling a child in a Catholic School, we are happy to send the certificate to the Church of Marriage or Catholic School on your behalf.
